- Supports to be delivered, line item codes, rates
- Gap fee arrangement (if applicable)
- Cancellation policy (2 clear business days)
- Participant rights and dispute resolution

Psychiatry + NDIS
NDIS does NOT fund psychiatric consultations. Functional capacity reports are billed as private fees.
