Registrar Activation Checklist
Registrar Scope of Practice
When activated, the Psychiatric Registrar may perform:
- General psychiatric assessment and management (within training scope)
- Prescribing under supervision (including S8 with appropriate state approvals)
- Medication reviews for stable patients
- Follow-up consultations for ongoing care
- Cannot independently sign reports as consultant psychiatrist
- All clinical decisions reviewed/supervised by FRANZCP supervisor

NEVER use Group A8 items (291, 296, 300–308) for the registrar. This will cause Medicare claim rejection.
Supervision Requirements
- Minimum 4 hours per week clinical supervision by FRANZCP holder
- Supervisor: A/Prof Charana Perera (MBBS, FRANZCP)
- Supervision can be remote (telehealth-compatible practice)
- Individual supervision: at least 1 hour per week (rostered, uninterrupted)
- Documented supervision log must be maintained for RANZCP 3GA placement
- Part-time (2 days/week): minimum 3 hours pro-rata supervision per week
Financial Model — 85% Split
Registrar receives 85% of the MBS schedule fee. Practice retains 15%.

Revenue Estimate (per day)
Assuming 6 × Item 57 (45–60 min consultations) per day:
- Total Medicare revenue: 6 × $61.00 = $366.00
- Registrar receives: 6 × $51.85 = $311.10
- Practice retains: 6 × $9.15 = $54.90
BBI payments (items 10990/10991) provide additional revenue — amount varies by MMM area.
Halaxy Configuration — Bulk Billing
- Add registrar as a new practitioner in Halaxy (Settings → Team)
- Enter Medicare provider number in practitioner profile
- Create appointment types for each duration (52, 53, 54, 57, 151)
- Set payment type to "Bulk Bill" for all registrar appointments
- Import Group A2 rebate amounts from MBS preset database (Finances → Fees → Update Fees)
- Set fee = schedule fee (no gap — patient pays $0)
- Enable automatic Medicare claiming
- Add video telehealth items (91792, 91803, 91804, 91805, 91923) as linked items
19AB Exemption — International Medical Graduates
If the registrar is an Australian citizen or permanent resident: no 19AB restrictions apply. The 3GA placement provides full Medicare access.
If the registrar is an International Medical Graduate (IMG):
- May require a 19AB exemption under the Distribution Priority Area (DPA) pathway
- Check with Services Australia before starting
- The 3GA program enrollment AND 19AB exemption may both be needed
